The SUNNY FUTURES Healthy Start project serves pregnant women in ten zip codes in the Sunnyside, Third and Fifth Ward communities of Houston, including:

Services Offered

Our program incorporates outreach, case management and health education in order to impact maternal and child health status, such as:
  • In-home case management to help pregnant and interconception women develop a plan to become self-sufficient
  • Counseling for postpartum depression
  • Referrals for primary care physicians, insurance enrollment, WIC, child care, GED, higher education, employment, social services, food pantry, clothing, utility assistance, child birth classes, substance abuse treatment, domestic violence prevention, and child developmental delays
  • Safe Riders car seat safety education provider and distribution site
  • Incentive program to encourage women to keep medical and in-home appointments

Outreach
SFHS’s Certified Community Health Workers provide community-based Outreach services including:
  • CHIP, Medicaid, Harris County Hospital District Gold Card, Women, Infant, Children (WIC) Program, Texas Women’s Medicaid Program Application Assistance
  • Referrals to local community resources
  • Healthcare navigation services
  • Car Seat Safety Education
     
Case Management
Our Certified Family Development Workers and Licensed Social Workers provide in-home services to eligible participants:
 
Health Education
Health education is the key to behavior modification. Our Health Education Coordinator promotes infant mortality awareness and healthy lifestyle choices in a variety of arenas:
  • Prenatal and Parenting Education Classes
  • Monthly “Mommy & Me” sessions for SFHS Program Participants
  • Health Education referrals
  • Breastfeeding Education

Mommy Money Incentive Program

To encourage involvement in SUNNY FUTURES Healthy Start, participants can shop at the Mommy Store using "dollars" earned by:

  • Attending any medical appointment for self or child
  • Keeping any home visit appointment
  • Attending SUNNY FUTURES Healthy Start events

The store includes many items, such as: 'Healthy

  • Infant clothes
  • Baby furniture
  • Infant care supplies (wipes, lotion, powder, soap, shampoo, diaper rash cream, detergent)
  • Toys
  • Bottles
  • Baby monitors
  • Walkers, bouncers and strollers
